Our Services



VNA of Albany offers a wide range of comprehensive services and programs to patients in upstate New York’s Capital Region. Referrals to VNA of Albany must be initiated by a patient’s primary care physician or specialist, or by a physician at a hospital or skilled nursing/sub-acute care facility.
Our services and programs are coordinated for patients by a VNA clinical case manager, who practices under the direction of physician’s orders.
The VNA offers skilled nursing; rehabilitation services including physical therapy, occupational therapy, speech language pathology; medical social services; and home health aide services.
We employ specialty care registered nurses who provide clinical expertise and oversight in: Cardiopulmonary diseases, Diabetes, and Infusion Therapy. An Asthma Program and Sub-Acute Transition Program are also provided to support hospital admission avoidance, patient teaching, positive patient outcomes, and care transitions. Skilled Nursing
VNA of Albany nurses provide skilled intermittent care at home to patients who may have recently been discharged from a hospital, skilled nursing facility or sub-acute facility; have a new diagnosis or multiple diagnoses, one or more chronic illnesses, change in medications, wound care needs, or requires education or monitoring at home between physician visits.
A VNA Case Manager will develop an individualized care plan specifically for each patient based on an assessment during the initial visit.
Skilled nursing services may include, but are not limited to:
- Wound care and ostomy
- Acute and chronic disease management and monitoring
- Infusion Therapy
- Monitoring during periods of medication adjustment and medication management
- Patient education related to disease process, medication, and treatment modalities
Rehabilitation Therapy
VNA of Albany provides rehabilitation services including physical therapy, occupational therapy and speech language pathology to patients who are recovering from surgery or have pain, deconditioning, or any type of cardiopulmonary, neuromuscular, neurological, or orthopedic deficit.
Our clinicians assess patients using industry-accepted fall risk assessment tools and implement a “Home Safety and Falls Prevention Program” with all our patients. Based upon the scores in these assessments, goals are established and interventions are identified. The VNA therapist may implement intensive targeted interventions such as exercises, safety measures, adaptive/supportive equipment evaluation, recommendations for vision/hearing/swallowing adaptations, medication reviews and follow-up, home environmental review and recommendations, teaching tools, and community resource identification.

Medical Social Work
Our medical social workers provide counseling and advocacy to patents about medical limitations, community resource education, and long-term planning. They are an integral part of the VNA’s interdisciplinary team of care providers. Medical social workers perform a comprehensive assessment of the patient’s social, emotional, environmental, and financial needs to help them and their families alleviate the hardships related to adverse health conditions. They also provide education on entitlements, community services, and health insurance coverage.
