VNA Services

VNA of Albany provides skilled nursing and rehabilitative services (physical therapy, occupational therapy and speech language pathology) to patients who reside in Albany, Columbia, Fulton, Greene, Montgomery, Rensselaer, Schenectady, Schoharie, Saratoga, Warren, and Washington counties.

All VNA of Albany services require a physician’s referral in order to initiate services. Our services are considered skilled (requiring the services of a nurse or therapist) and intermittent.

Referrals to VNA of Albany are sent either by a patient’s primary care physician or specialist; or by a physician at a hospital or skilled nursing/sub-acute facility.

VNA of Albany has on-site home care liaison RN’s at Albany Medical Center, Columbia Memorial Hospital, Glens Falls Hospital, and Saratoga Hospital who work directly with the hospital’s case management and medical staff to facilitate a smooth and coordinated transition to VNA care. Our liaison RN’s meet with patients and their families to explain our services, discuss expectations for care at home, and answer questions.

In addition, VNA of Albany has community liaison RN’s who visit many of the region’s skilled nursing/sub-acute facilities and provide support to patients and staff members.
