Visiting Nurses Home Care (VNHC), located at 28 Colvin Ave Albany, NY, is a licensed home care service agency founded in 1988. VNHC provides supportive services including certified home health and personal care, licensed practical nursing, and registered professional nursing. VNHC provides service coordination for the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Programs, and is a provider for the Consumer Directed Personal Assistant Program (CDPAP).

VNHC is licensed to provide services in 18 counties including: Albany, Clinton, Columbia, Delaware, Dutchess, Essex, Franklin, Fulton, Greene, Hamilton, Montgomery, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Warren and Washington. In addition to the main office in Menands, VNHC has satellite offices in Catskill, Cobleskill, Glens Falls, and Johnstown.

VNA Foundation was established to aid and assist VNA of Albany. Foundation funds are utilized to provide home care services, durable medical equipment, and other necessities to VNA patients experiencing financial hardship. Funding sources for the Foundation include annual appeals; donations from patients and/or family members in memory of a loved one or in honor of their nurse or therapist; employee donations, and grant solicitations.
