VNA of Albany (VNA), located at 35 Colvin Avenue, Albany NY, is a certified home health care agency providing skilled nursing care, rehabilitative services (physical, occupational, speech pathology therapies), and medical social work to individuals in the comfort of their homes. These individuals have conditions that are acute or chronic and require the skill of our dedicated clinicians. VNA of Albany provides care to residents in 11 Capital Region counties: Albany, Columbia, Fulton, Greene, Montgomery, Rensselaer, Saratoga, Schenectady, Schoharie, Warren and Washington.
The mission of the VNA is to provide health care services to patients in accordance with the highest ethical and professional standards in order to restore, promote, maintain, and support each of our clients’ health and well-being.
Founded in 1880, VNA of Albany has a long standing history of meeting community home care needs. Incorporated in 1897 as a 501 (c) (3) not-for-profit corporation, VNA of Albany has been a staple in the region as a community based health care provider and an employer. The VNA has an affiliation arrangement with several area hospitals including Albany Medical Center, Columbia Memorial Hospital, Glens Falls Hospital, and Saratoga Hospital. These facilities collaborate with the VNA on clinical protocols and patient education to ensure an effective transition of patients to home care. VNA Liaison RNs work on-site at the hospitals to coordinate patients’ transition to home care and to promote positive outcomes.

Visiting Nurses Home Care (VNHC), located at 28 Colvin Ave Albany NY 12206, is a licensed home care service agency founded in 1988. VNHC provides supportive services including certified home health and personal care, licensed practical nursing, and registered professional nursing. VNHC provides service coordination for the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Programs, and is a provider for the Consumer Directed Personal Assistant Program (CDPAP).
VNHC is licensed to provide services in 18 counties including: Albany, Clinton, Columbia, Delaware, Dutchess, Essex, Franklin, Fulton, Greene, Hamilton, Montgomery, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Warren and Washington. In addition to the main office in Menands, VNHC has satellite offices in Catskill, Cobleskill, Glens Falls, and Johnstown.

VNA Foundation was established to aid and assist VNA of Albany. Foundation funds are utilized to provide home care services, durable medical equipment, and other necessities to VNA patients experiencing financial hardship. Funding sources for the Foundation include annual appeals; donations from patients and/or family members in memory of a loved one or in honor of their nurse or therapist; employee donations, and grant solicitations.
