Nutritional Counseling Services
& IV Infusion Therapy

All VNA patients receive a nutritional assessment by the admitting staff person.  A referral for a VNA Registered Dietitian nutritional evaluation requires physician’s orders.

The dietitian provides:

  • Assessment and monitoring of patient’s nutritional status
  • Individual and family/caregiver education
  • Recommendations for dietary change
  • Active consultation in treatment plan with patient, caregiver, VNA case
  • Manager and patient physician

Nutritional Risk Factors

  • Mental confusion affecting eating
  • Missed meals, snacks, fluids
  • Food wastage or rejection
  • Those who must make major diet changes, ie, morbidly obese, anorectics, CHF, diabetics      
  • Depression, loneliness, bereavement
  • Poor therapeutic diet knowledge
  • Limited dietary variety

High-tech IV home infusion therapies available from the VNA of Albany include parenteral nutrition, antibiotics, chemotherapy, and, as appropriate, RN instructions of patient/caregiver with administration and line care.
