Life Stability Programs

 

Community Mental Health and MSW Services

Some of the problems that could mean a patient may benefit from these VNA services:

  • Psychiatric diagnosis, eg, paranoid schizophrenia, acute depression, Alzheimer’s dementia, grief reaction
  • Hallucinations/delusions, fears/anxiety/phobias, substance abuse, self-destructive behaviors, recent changes in memory, alertness, orientation, personality
  • Secondary diagnosis in which manifestations are a problem for patient/family/caregiver
  • Patient receives psychotropic medications. Crisis intervention is needed
  • Problems limit effective treatment, rate of recovery, optimum health level
  • Patient needs help with long-term planning, access to other community services, obtaining SSI, food stamps, Medicaid, medical supplies, equipment, transportation

VNA of Albany staff can assist providers and patients to define Medicare homebound status for potential Community Mental Health Services patients. 

Geriatric Alcohol Services

  • Specialized program for individuals 50 years or older
  • Alcohol and substance abuse intervention, education and prevention
  • Assessment, referral and education
  • Nurses with expertise in alcohol and substance abuse in the elderly population

Personal Assistive Devices

  • 24-hour personal emergency response systems (monthly rentals)
  • Personal medication dispensing system, built-in monitoring capability (monthly rentals)
