Home Safety & Falls Reduction

Our program goal is to reduce and prevent falls that result in injury.

94% of patients completing this program had fewer falls than they did
60 days prior to receiving VNA services.

Common Risks:

  • History of at least one fall within the past year, or a chronic fear of falling
  • Difficulty walking, getting up or sitting down
  • Need to improve endurance, strength, fitness and balance
  • Sudden change in blood pressure
  • Improper footwear
  • Need for medication review, including evaluation for side effects
  • Need for assistive devices in the home
