Home Safety & Falls Reduction
Our program goal is to reduce and prevent falls that result in injury.
94% of patients completing this program had fewer falls than they did
60 days prior to receiving VNA services.
Thorough risk assessment and intensive targeted interventions:
- Exercises
- Safety measures
- Adaptive/supportive equipment evaluation
- Medication review and follow-up
- Recommendations for vision/hearing adaptations
- Medication review and follow-up
- Preventive safety interventions
- Home environmental review and recommendations
- Teaching tools
- Community resources
- Post-discharge telephone survey
Common Risks:
- History of at least one fall within the past year, or a chronic fear of falling
- Difficulty walking, getting up or sitting down
- Need to improve endurance, strength, fitness and balance
- Sudden change in blood pressure
- Improper footwear
- Need for medication review, including evaluation for side effects
- Need for assistive devices in the home
