We Help Our Diabetes Patients to:
- Verbalize disease process and actions to take to manage diabetes
- Recognize signs/symptoms of complications and actions to take to manage complications
- Manage insulin injection safely; or take oral medication correctly
- Adhere to dietary prescription
- Demonstrate ability to monitor blood glucose and take appropriate action in response to results
- Practice daily foot care/skin integrity intact
- Prevent, recognize and respond to hypoglycemia
- Demonstrate blood glucose within target range
- Identify when to contact MD
