VNA Home Health provides skilled nursing, and rehabilitative services (physical therapy, occupational therapy and speech language pathology) to patients who reside in the counties of Albany, Columbia, Fulton, Greene, Montgomery, Rensselaer, Schenectady, Schoharie, Saratoga, Warren and Washington.
All VNA Home Health services require a physician’s referral in order to initiate services.
VNA Home Health services are considered skilled (requiring the services of a nurse or therapist) and “intermittent.”

Referrals to VNA Home Health are sent either by a patient’s primary care physician or specialist; or by a physician at a hospital or skilled nursing/sub-acute facility.
VNA Home Health has on-site home care liaison RN’s at Albany Medical Center, Columbia Memorial Hospital, Glens Falls Hospital and Saratoga Hospital who work directly with the hospital’s case management and medical staff to facilitate a smooth and coordinated transition to VNA Home Health. Our liaison RN’s will meet with the patient and their families to describe VNA services, discuss expectations for care at home, and answer questions.
In addition, VNA Home Health has community liaison RN’s who visit many of the skilled nursing/sub-acute facilities and provide support to patients and staff members.
