Transition Coach
Full Time Position for RN
- Acts as a liason between hospital and homecare agency in transitioning patients to homecare services
- Provides patient care
- Minimum one year recent cardiac and pulmonary experience required
- Strong clinical skills required
- IV & Homecare experience preferred
- BSN preferred
- Must have valid NYS drivers license and automobile
- Requires weekend, on-call and holiday rotations.
Registered Professional Nurses
Full Time 8am-4pm or
Part Time (Minimum 3 days per week)
- Minimum one year recent medical/surgical experience
- Strong clinical skills required
- IV & Homecare experience preferred
- Must be a graduate of an accredited school of nursing
- Must have valid NYS drivers license and automobile
- Requires weekend, on-call and holiday rotations.
Registered Nurse Per Diem/Per Visit
Flexible per diem positions available
2 or 3 days per week and 1 weekend per month
OR
2 weekdays and 2 weekends per month
- Minimum one year recent medical/surgical experience
- Strong clinical skills required
- IV & Home Care experience preferred
- Must be a graduate from an accredited school of nursing
- Must have valid NYS drivers license and automobile
- Holiday rotations
- Must be available for 4 to 6 weeks for orientation

12pm-8pm or 1pm-9pm (No Weekends Required)

Maternal Child Health Nurse (Per Diem)
- Experience with antepartum, postpartum & pediatric nursing care.
- Advanced clinical practice skills
- IV & Home Care experience preferred.

Psychiatric Nurse
Full or Part Time Position
- Minimum one year recent pysch/medical/surgical experience.
- Strong clinical skills required
- IV & Home Care experience preferred

Certified Wound Ostomy Continence Nurse
Full or Part Time Position
- Minimum one year recent medical/surgical experience
- Strong clinical skills required
- IV & Home Care experience preferred
- Must be a graduate from an accredited school of nursing
- Provides advanced wound care/ostomy/skin care clinical practice
- Staff and patient education and consultation.
- Must be a graduate from an accredited school of nursing
- Must be certified or actively pursuing certificate as an Enterostomal Therapist.
- Must have automobile and valid NYS drivers license.
