I work with case managers who coordinate patient care and do all of the physical pieces—dressing changes, vital signs, medications management, and IV therapies, as well as a state-of-the-art telehealth program, and our new electronic patient record system—but home care with the VNA also allows us to work with patients to teach them to take better care of themselves, make healthy life changes, and communicate clearly with their physicians.
