VNA of Albany (VNA) is a certified home health care agency providing skilled nursing care, rehabilitative services (physical, occupational, speech pathology therapies), and medical social work to individuals in the comfort of their homes. These individuals have conditions that are acute or chronic and require the skill and compassion of our dedicated clinicians. VNA of Albany provides care to residents in 11 Capital Region counties: Albany, Columbia, Fulton, Greene, Montgomery, Rensselaer, Saratoga, Schenectady, Schoharie, Warren and Washington.
The mission of the VNA is to provide health care services to patients in accordance with the highest ethical and professional standards in order to restore, promote, maintain, and support each of our clients’ health and well-being.
Founded in 1880 by a small group of public spirited citizens concerned about the lack of home care services in the Albany NY area, VNA of Albany has a long standing history of meeting community home care needs. Incorporated in 1897 as a 501 (c) (3) not-for-profit corporation, VNA of Albany has been a staple in the region as a community based health care provider and an employer.
The VNA has an affiliation arrangement with Albany Medical Center (AMC), the only tertiary medical teaching facility within a 100-mile radius of Albany NY. The VNA and AMC collaborate on clinical protocols and continuity in patient teaching to ensure an effective transition of patients to home care, and promote positive outcomes. VNA Liaison RNs work on-site at AMC to coordinate patients’ transition to home care. These liaisons are also on-site at Saratoga, Glens Falls, and Columbia Memorial Hospitals, serving a similar function.

Visiting Nurses Home Care
Visiting Nurses Home Care (VNHC) is a licensed home care service agency founded in 1988 and is an affiliate of VNA of Albany. VNHC provides supportive services including certified home health and personal care, homemakers, companions, licensed practical nursing, and registered professional nursing. VNHC provides service coordination for the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Programs, and is a provider of the Consumer Directed Personal Assistant Program (CDPAP).
VNHC provides services in 18 counties including: Albany, Clinton, Columbia, Delaware, Dutchess, Essex, Franklin, Fulton, Greene, Hamilton, Montgomery, Otsego, Rensselaer, Saratoga,, Schenectady, Schoharie, Warren and Washington. The main office is located at 150 Broadway Menands, NY; with satellite offices in Glens Falls, Saratoga, Johnstown, Catskill and Cobleskill.

VNA Foundation was established to aid and assist VNA of Albany. Foundation funds are utilized to provide home health care services to the medically indigent patients served by VNA of Albany. Funding sources for the foundation include annual appeals; donations from patients and/or their family members in memory of a loved one or in honor of their nurse or therapist; employee donations, and grant solicitations.
